One reason for this could be a blockage in the veins that supply blood to your legs. An easy, painless test to find out if you have such conditions is the Ankle-Brachial Index (ABI) test. Let's talk about this in detail today.
What is the Ankle-Brachial Index (ABI)?
Simply put, this ABI is a simple test that measures blood pressure in your ankle and upper arm. It mainly checks whether the large blood vessels in your legs, the arteries, have become blocked or have reduced blood flow due to fatty deposits (like dirt clogging a water pipe). Doctors call this condition ``Peripheral Artery Disease`` or PAD. In Sinhala, it means peripheral artery disease.
This PAD condition can cause the legs to not receive enough oxygen and nutrients. That's when the symptoms I mentioned earlier, such as leg pain and numbness when walking, appear. So the ABI test can determine whether you have PAD and, if so, how severe it is.
When should I get this ABI test done?
Not everyone needs to have this test, but a doctor may suggest an ABI test if you have any of the following:
- Leg pain: If you experience pain, stiffness, or a feeling of weakness in your legs (thighs, calves), especially when walking or exercising, and it subsides after sitting for a while and resting.
- If you are 65 years of age or older: The risk of developing PAD increases with age.
- If you are between 50 and 64 years old and have other risk factors:
- If you have `(Diabetes)`.
- If you have ``(High blood pressure)``.
- If you have high cholesterol in your blood.
- If you are a tobacco user (such as cigarettes, beedis).
- If anyone in your family (mother, father, siblings) has had PAD before.
- Even if you are under 50 years old, you have diabetes and one other risk factor (from those mentioned above).
Imagine, this ABI test can identify PAD before it gets worse. Then, treatment can be started quickly and it will be a great help in preventing conditions like ``Critical limb ischemia`` and ``Amputation``, which can cause serious damage to the legs, sometimes to the point of amputation.
How is this ABI test done?
This is very simple. It's like having your blood pressure checked when you normally go to the doctor. But this time , they measure the blood pressure in your arms, your ankles, and all four legs.
This test is usually performed by a nurse.
1. First, a blood pressure cuff is placed on one of your upper arms, tightened slightly, and blood pressure is measured as you inflate and deflate.
2. Then measure the other arm in the same way.
3. Next, the cuff is placed around the ankle of one of your legs and the blood pressure is measured there as well.
4. Measure the other leg in the same way.
When measuring your blood pressure, your healthcare provider uses a small ultrasound machine (also called a handheld Doppler). This listens to the sound of blood flowing through your brachial artery, dorsalis pedis artery, or posterior tibial artery. The pressure gauge reads the sound when it is heard.
The most important thing is that this is a `noninvasive` test. That means there are no needles or cuts in the skin. So there is nothing to be afraid of. It is very easy.
How should I prepare before the ABI test?
To make the test easier for you and get accurate results, you need to do a few things:
- Do not drink anything containing caffeine (coffee, tea, some soft drinks) on the day of the test.
- Avoid using tobacco (cigarettes, beedis) and drinking alcohol for one to two hours before the test.
- Do not do any exercise for an hour before the test. Also, limit your walking.
- Wear comfortable, loose clothing that allows you to easily expose the tops of your arms and ankles.
- Before going for the test, urinate thoroughly and empty your bladder. This will help you feel comfortable during the test.
Another important thing: If you have previously had any surgery to improve blood flow in your legs (such as vascular bypass), you should definitely tell the person doing the test about it, as it may affect the way the test is performed.
What to expect during the test?
When you go to the examination room, you will be asked to lie down and relax for 10 to 30 minutes before the test begins. This is to help your body calm down and your blood pressure stabilize. You will need to lie down with your arms and legs straight throughout the test. Sometimes, small pillows will be placed under your arms and ankles, raising them to the level of your heart. This will allow for accurate blood pressure readings.
Then, as I said before, the medical worker:
1. Your blood pressure will be measured in one arm.
2. Then, the blood pressure is measured in the ankle of the leg on the same side as that arm.
3. Next, the blood pressure is measured in the ankle of the other leg.
4. Finally, the blood pressure is measured in the other arm.
Sometimes, if you want to get a more accurate reading, you can take this measurement more than once. Your healthcare provider will tell you about this. The entire process of measuring blood pressure usually takes a very short time, about 10 to 20 minutes. That's it, the test is over!
Are there any risks with this test?
The ABI test is a very safe test. There are generally no risks or side effects.
However, if you have any of these conditions, be sure to tell your doctor in advance:
- If you have unhealed wounds or ulcers on your legs: It is not good to apply a cuff to such an area, so the medical staff will either avoid that area or use another method.
- If you have recently had vascular bypass surgery in your leg: Cuffing such an area can be dangerous and can lead to blood clots.
When these things are said, the medical team takes the necessary steps.
What happens after the test?
You can resume your normal activities as soon as the test is done. No rest is needed. Sometimes the medical staff who performed the test will be able to give you some feedback on the results. Otherwise, you will need to meet with the doctor who ordered the test to discuss the results.
How is this ABI value calculated?
Okay, now let's see how to calculate this ABI value. This may seem a bit mathematical, but it's actually quite easy.
Simply put, the highest of your two ankle blood pressure readings (systolic - the top number of your blood pressure readings) is divided by the highest of your two arm blood pressure readings. That's what the ABI is.
For example, consider:
- Your blood pressure in your left ankle is 105/65 mmHg, and in your right ankle it is 100/60 mmHg.
- The blood pressure in your left arm is 130/85 mmHg, and in your right arm it is 125/80 mmHg.
Now to find the ABI value, we take the highest of the numbers (`(systolic)`) taken from each place.
- Highest ``(systolic)`` value from ankle: 105 mmHg
- Highest `(systolic)` value from the hands: 130 mmHg
Now we need to divide the ankle value (105) by the hand value (130).
105 ÷ 130 = 0.807 (approximately 0.8)
So, in this example, the ABI value is 0.8.
What do these results say?
The ABI value you receive gives an idea of the condition of the blood vessels in your legs.
- 1.0 to 1.3: This is a normal value. It means that your legs have good blood circulation.
- 0.9 to 1.0: This is a borderline value. It means that PAD is on the verge of developing. Lifestyle changes may be recommended.
- 0.7 to 0.9: This is a mild PAD condition.
- 0.4 to 0.7: This is a moderate PAD condition.
- If it is less than 0.4: This is severe PAD . At this point, there may be more symptoms.
If the ABI value is 0.9 or less, it means you have PAD.
Sometimes the ABI valueIt can be higher than 1.4. If so, it means that your blood vessels (arteries) have become a little hard, ``calcified`` (like stones). This can happen as you get older, in people with ``diabetes`` (diabetes), or in people with kidney disease. When the veins are hard like this, it is difficult to tell for sure whether you have PAD with the ABI test. If so, your doctor will order other tests (for example, the ``Toe-Brachial Index - TBI)`` test - which measures blood pressure in the big toe).
Research has found that people with an ABI greater than 1.4 have a higher risk of death from cardiovascular disease. So, whether the value is low or high, if it is outside the normal range, it is important to talk to your doctor about it.
When should I call my doctor ?
If you don't get your results soon after your ABI test, be sure to inform your doctor.
Depending on your ABI results, your doctor may recommend further testing, treatment, or lifestyle changes (such as diet, exercise, and smoking cessation).
If your ABI is outside the normal range, your doctor may order repeat ABI tests from time to time to see if your PAD is getting worse or if it is getting better with treatment.
