When you get a breast exam, whether it's a mammogram or an ultrasound, and you get the report, you might feel a little scared and curious when you see the words and numbers on it. Especially if it says 'BI-RADS' with some number, you might think, 'Oh, what's this?' It's actually normal. But don't worry. Today we're going to talk about what BI-RADS is, what it really means, and how it can affect your health.
What is BI-RADS simply?
Simply put, BI-RADS is an acronym for “Breast Imaging-Reporting and Data System.” This is a standard system that radiologists use to describe your breast exam reports. Imagine, no matter where you are in Sri Lanka or anywhere in the world, everyone speaks the same language. This system uses the tests you have performed, which means:
- Mammography
- Breast Ultrasound
- MRI scan
Report the results of tests such as.
There were two main reasons for introducing this system.
1. Bring a single standard to reporting: No matter where a test is done in Sri Lanka, it is reported in the same way. This way, there will be no confusion when sending your details from one doctor to another. Everyone can easily understand it.
2. Clearly communicate breast cancer risk: This method provides an easy-to-understand classification of your risk of developing breast cancer.
Your report may not have the BI-RADS number listed in bold. However, it will be included in the full report your doctor receives. You can ask your doctor about it.
BI-RADS classification and its meaning
The BI-RADS system has a classification from 0 to 6. Each of these numbers tells a different story. Let's take a look at what each one means, and what to do next in such a situation.
| BI-RADS number | Meaning | What to do next |
|---|---|---|
| BI-RADS 0 | The test is incomplete. This means that a conclusion cannot be reached because the images received are not clear or further information is needed. This does not mean that there is anything wrong. | You may need to compare it with your old mammogram report or have additional tests (e.g., another mammogram, ultrasound scan). |
| BI-RADS 1 | No problem (Negative) This is the best result. It means that there are no suspicious lumps or changes in your breasts. | Get regular screening at the appropriate time based on your age and risk. |
| BI-RADS 2 | Benign Finding This also means that there is no cancer. However, there are some non-cancerous things (e.g. cysts, fibroadenomas) that are seen. These are not harmful. | This is also a normal condition. Get a routine checkup at the appropriate time. |
| BI-RADS 3 | Probably Benign What you see in this is 98% unlikely to be cancer. But there is a slight risk because it is not 100% certain. | The doctor will often recommend a short-term follow-up in about 6 months to see if things have changed. |
| BI-RADS 4 | Suspicious This is the number that scares a lot of people. But it doesn't mean you have cancer. It just means "it's suspicious, so let's look into it further." | The doctor will most likely recommend a biopsy , which involves taking a sample of tissue for examination. |
| BI-RADS 5 | Highly Suggestive of Malignancy Based on the symptoms seen in this case, the likelihood of cancer is very high (more than 95%). | A biopsy is definitely needed to confirm. |
| BI-RADS 6 | Known Biopsy-Proven Malignancy This is obtained during a test performed before treatment (e.g., before surgery) after a previous biopsy has confirmed that cancer is present. | Further work will be carried out according to the treatment plan prescribed by the doctor. |
What are the advantages of the BI-RADS system?
This method is very useful for both doctors and patients.
- Standardization: When a doctor in Colombo, Sri Lanka, and a doctor in Jaffna say BI-RADS 4, it means the same thing. This greatly reduces the chances of errors and confusion in understanding your reports.
- Risk Assessment: This classification gives you a rough idea of your risk of developing cancer, which helps your doctor decide how soon you need to do further testing.
- Efficiency: Instead of having to describe each report in detail, this saves radiologists time by categorizing them.
Are there no drawbacks to this method?
Like any method, this one has a few small limitations.
- Subjectivity: A human being classifies a scan image. So, to a very small extent, what one radiologist sees may differ slightly from what another radiologist sees.
- False Results: Although very rare , sometimes a test can report a suspicious result (false positive) when there is no cancer, or a test can report a positive result (false negative). This can cause unnecessary concern or delay diagnosis. However, keep in mind that this is a very rare occurrence.
- Lack of specific information (Unspecific): The BI-RADS number only tells you whether there is something suspicious or not. It does not provide in-depth information about what kind of suspicious area is there, what its nature is. That is why tests like a biopsy are needed.
Does BI-RADS 4 mean cancer?
This is the biggest question many people have. The answer is, no.
BI-RADS 4 does not mean that you have cancer. It means that the results of your scan are "suspicious." This means that it may or may not be cancer. The risk of cancer can range from very low to high. That's why a biopsy is done to find out for sure.
BI-RADS 4 doesn't mean you have cancer. It means, 'We need to look into this further, so let's move on to the next step.'
So if your report says BI-RADS 4, don't worry unnecessarily. It just means that your doctor will refer you for further testing.
Take-Home Message
- BI-RADS is a globally accepted standard method used to classify the results of your breast examination reports (mammogram, ultrasound).
- This number tells you what your cancer risk is and what steps you should take next.
- BI-RADS 1 and 2 mean good results with no problems.
- Even if you get a high score like BI-RADS 4 or 5, it doesn't necessarily mean you have cancer. It just means 'it's suspicious and needs further testing.'
- If you have any questions about the BI-RADS number on your report, don't panic and talk to your doctor about it. Only he or she can explain exactly what it means for your situation.
