Today we are going to talk about a very important topic. You may have heard of a drug called `(Opioid)`. These can be the drugs that doctors sometimes prescribe to control severe pain, or they can be things like `(Heroin)` or `(Fentanyl)` that some people unfortunately use as a drug. However, if these `(Opioid)` things are taken in too much, that is, if they become an `(Overdose)`, it can be very dangerous for life. But, did you know that, if used at the right time, there is a little medicine that can save someone from such an `(Opioid Overdose)`? That medicine is called `(Narcan)`. Today, we will talk about this `(Narcan)` in detail.
What is Narcan? Simply put...
Narcan is actually a brand name. The active ingredient in it is called Naloxone. Think of it like an emergency device used in a life-saving operation. If someone has overdosed on an opioid, such as heroin or fentanyl, or an opioid painkiller prescribed by a doctor, Narcan can save their life.
The most important thing is that Narcan is a drug that only works for opioid overdoses. It has no effect on other types of drug overdoses.
How do you use Narcan? Is it difficult?
Absolutely not! This is one of the biggest advantages of Narcan. You don't need any medical training or special knowledge to use Narcan. It's very simple.
There are generally two types of Narcan:
- Nasal Spray: This is the most common type. It is just like a regular nasal spray. It is very easy to use. There are only a few instructions.
- Injection: This is also sometimes seen, but is not as common as the nasal spray.
You can learn how to use these by watching videos online. For example, the CDC in America has released videos about this. So there is nothing to worry about. This is designed to be portable and quick to use in an emergency.
What actually happens with Narcan?
Okay, now let's see what happens when Narcan enters the body.
Simply put, when you take too much of an opioid, that is, when you overdose, the drug binds to certain parts of your brain, especially the centers that control breathing. That is the main reason why you suffocate, stop breathing, and die from an opioid overdose.
What Narcan does is it removes the opioids from the places where they are bound in the brain. Think of it like a key, stuck in a lock in the brain called a receptor. Narcan is another powerful key that removes the opioid key and releases the lock. It starts working within a few minutes. Then the breathing that had been interrupted starts to return to normal. In other words, Narcan is like a rope that pulls someone who is between life and death back to life in the event of an opioid overdose.
Who should have this Narcan?
This is a very important question. Since Narcan is an emergency drug, it is very important for some people to have it on hand. Especially:
- People who are addicted to opioids and are trying to quit: These people are more likely to unknowingly relapse and overdose. Therefore, it is very important for them and their family members to have Narcan on hand.
- People who use prescription opioid painkillers: Not everyone, but some people are at higher risk of overdose because they take opioid painkillers in high doses or have other medical conditions. Also, the risk of overdose is even higher when opioids are used together with benzodiazepines (e.g., tranquilizers like diazepam and clonazepam). Both of these medications affect the centers that control breathing. So, if you are taking these medications, it is wise to talk to your doctor about keeping Narcan on hand.
- People who are close to people who use opioids: If you have a friend, family member, or someone you know who is using opioids, keeping Narcan with you as an aid can help save their life in an emergency.
Imagine you have a friend who is trying to recover from an opioid overdose. One day, while you're with him, he suddenly becomes unconscious and can't breathe. If you have Narcan with you, you can quickly administer it and help save his life until the ambulance arrives.
Where can I get Narcan?
In countries like the United States, Narcan nasal spray is available over the counter at a pharmacy without a prescription. Narcan is also available, sometimes free of charge, through local or state programs, harm reduction locations, or community organizations . It is completely legal to obtain, carry, and use Narcan in an emergency. In Sri Lanka, it is best to ask your doctor or health authorities about this situation.
How do you know for sure if someone has overdosed?
This is also very important. Recognizing the symptoms of an opioid overdose is the only way to act quickly. If someone has overdosed on opioids, they may show symptoms like these:
- The pupils of the eyes become smaller, to be precise, like the tip of a pen (Pinpoint Pupils).
- Suddenly, you lose consciousness and fall down.
- Breathing becomes very slow, or breathing stops completely.
- The entire body becomes lifeless, hanging down like a piece of cloth (Limp body).
- Lips and fingernails turn blue. This is because the body is not getting enough oxygen.
- A strange sound comes from the throat, like choking or gurgling sounds.
If one or more of these symptoms appear at once, there is a high chance that it is an ``Opioid Overdose''.
What do I do if someone overdoses?
Okay, now you know the signs of an overdose. What should you do if you see someone like that? Don't panic. Follow these steps:
1. Call the emergency services immediately (1990 in Sri Lanka, 911 in the US). Tell them where you are and the patient's condition.
2. If you have Narcan, give it. Either spray it into the nose or inject it as directed.
3. Turn the patient onto their side and have them lie down in the ``Recovery position''. This will help prevent the vomit from getting stuck in their throat if they vomit.
4. Try to keep the victim as conscious as possible. Talk to them, pat them on the body. If they appear to have stopped breathing, give them rescue breathing if you can. If they appear to have stopped breathing, start cardiopulmonary resuscitation (CPR) if you can.
5. Stay with the patient until the ambulance arrives. Do not leave him alone.
Very important: Sometimes you may not know for sure if someone has actually overdosed. However, if you have the slightest doubt, treat it as an overdose and act accordingly. Narcan is a drug that will not harm someone who is not already in an opioid overdose. So, if in doubt, don't be afraid to give it!
What other important things do you want to know?
Many countries have "Good Samaritan Laws." This means that if you go to help someone who has overdosed and call emergency services, neither you nor the person who has overdosed will face legal action. The hope is that people will come forward without fear and help save a life.
Where can I find help for myself or someone I know?
If you or someone you know is struggling with drug addiction, remember that recovery is possible. It can be a difficult journey to do alone, but it's never too late to ask for help.
- Talk to your family doctor.
- Check the websites of your local health department or contact them.
- In the US, there are phone numbers like 1-800-662-HELP (4357). You can also get more information and support from websites like `CDC.gov` and `SAMHSA.gov`. There are places in Sri Lanka that provide similar services, so check them out.
Finally, things to remember (Take-Home Message)
Okay, so we've talked a lot about Narcan today. It's important to keep this in mind:
- Narcan is a valuable drug that can save a life from an opioid overdose.
- It is very easy to use and does not require special medical training.
- It is important to recognize the symptoms of an opioid overdose (small pupils, loss of consciousness, difficulty breathing).
- If you suspect someone has overdosed, don't hesitate to give Narcan and call emergency services immediately.
- Drug addiction is a disease, and there is treatment for it. Don't be afraid to ask for help.
If you educate someone else with this knowledge, it may even help save a life of someone in need. Let's all be aware of this and act responsibly.
