Do you often have stomach aches and bloating? Sometimes you feel like your stomach is about to burst, and you also feel the need to go to the bathroom. You know how annoying these kinds of intestinal problems can be, especially if a doctor tells you that you have a condition like ``Irritable Bowel Syndrome - IBS''. So today we are going to talk about a special medicine that is given to control these kinds of stomach and intestinal diseases.
What is this medicine? Let's understand it simply.
This is not a single drug, but a combination of four drugs. That is, a solution or pill made from the four drugs Phenobarbital, Hyoscyamine, Atropine, and Scopolamine . In Sri Lanka, you may find this under brand names like `Donnatal` and `Antispasmodic`.
Simply put, this medicine works by relaxing the muscles in your stomach and intestines. Think of it like when your intestinal muscles contract too tightly (cramps), you get pain. This medicine controls those contractions, reducing pain and discomfort.
This is mainly used for situations like these:
- Stomach Ulcers
- Irritable Bowel Syndrome (IBS)
- Bowel Inflammation
Things you need to tell your doctor before taking this medicine

How should I use the medicine?
This medicine is to be taken by mouth. It is very important to take it exactly as prescribed by the doctor, at the same time, and in the same amount.
- Correct measurement: If this is a liquid, use a special marked spoon or a `dropper` that allows you to measure it. Do not use household teaspoons or tablespoons, as they are not accurate. This can cause the dosage to fluctuate.
- On time: Take your medicine exactly at the prescribed times. Do not take more than the number of times your doctor tells you to.
- Stopping: Do not stop taking the medication suddenly without your doctor's advice.
For young children and the elderly
Special care is needed if this medicine is given to young children. Also, if you are over the age of 65 , your body may react a little more severely to this medicine. Therefore, a lower dose is usually prescribed.
Warning! Overdose
If you think you have taken too much medicine, go to the nearest hospital Emergency Department (ETU) immediately. Or call the National Poisons Information Centre at the Colombo National Hospital for advice. Treat this as an emergency.
What if I miss a dose of medication?
If you forget to take a dose, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and take your next dose at the scheduled time. Never take a double dose to make up for a forgotten dose.
Other medications that may interact with this medicine
It is very important to tell your doctor about all other medications you are taking (medicines for other conditions, vitamins, traditional medicines, herbal teas) while taking this medicine, as some medications may interact with it.
Especially do not take it with these medications:
- Some medications for HIV (e.g. atazanavir, cobicistat, darunavir)
- Voriconazole (an antifungal)
It can also interact with blood thinners like warfarin. This list is not complete. So tell your doctor about everything you take. Also, tell him if you smoke or drink alcohol.
Things to keep in mind when taking medication
You may experience some changes in your body while taking this medication. It is important to be aware of this.
- Dry mouth: Dry mouth is common. You can control this by chewing sugarless gum, sucking on a candy bar, and drinking plenty of water.
- Dry eyes and blurred vision: Your eyes may feel dry and slightly blurry. This may be uncomfortable if you wear contact lenses. Ask your doctor about lubricating drops.
- Drowsiness and dizziness: This medicine can cause drowsiness and dizziness . So avoid driving, operating machinery, or doing anything that requires alertness until you know how it affects you. Especially if you are elderly, do not stand up or sit up suddenly. Do these things slowly.
- Alcohol: Avoid drinking alcohol completely. This medication can interact with alcohol, increasing drowsiness and other side effects.
- Birth control: This is very important. If you are taking hormonal birth control pills (Estrogen/Progestin), taking this medication may reduce the effectiveness of those pills. Therefore, talk to your doctor about using another barrier method of birth control, such as a condom.
- Serious skin reactions: Although rare, this medicine can cause serious skin reactions. If you develop symptoms such as fever, chills, and red or purple skin rashes that later turn into blisters and start to peel, tell your doctor immediately.
- Avoid excessive heat: This medicine reduces sweating. Therefore, when in extremely hot places (e.g. saunas, hot tubs), the body temperature can rise dangerously and cause `Heat Stroke`. So be careful.
