Do you sometimes feel like when you try to move an arm or leg, it suddenly becomes stiff as if it has been hit by a rock? Or do you feel like your muscles are twitching and in pain even when you are just standing still? This is a condition that many people experience, but do not know exactly what it is. In medicine, we call this Spasticity. Don't worry, today we will talk about everything very simply, in a way that you can understand.
Simply put, what is Spasticity?
Spasticity is a disorder of the system that controls the movement of our muscles. Think about it, when we normally bend an arm, one group of muscles tightens (contracts), while the other group of muscles relaxes. It's like a tug-of-war; when one side is pulled, the other side should relax. This process is controlled by our central nervous system, which is the brain and spinal cord (the nerve cord inside the spinal column) .
If this central nervous system is damaged for some reason, the signal system to those muscles becomes disrupted. What happens then is that when you try to move an arm, the muscles that should be tight and the muscles that should be loose both tighten at the same time. This makes it difficult to move, causes muscle stiffness, and can cause pain.
For some, this condition may be mild, just a slight muscle tightness. For others, it can progress to a condition where the limb becomes so tight that they can't be controlled, causing severe pain.
| Characteristic | Simple explanation |
|---|---|
| Increased muscle tone (Hypertonia) | Even when you're just standing, your muscles are tight and not loose. |
| Muscle spasms | A sudden, uncontrollable contraction and tightening of muscles. |
| Clonus | A series of rapid, shaking-like jerks in places like the ankles. |
| Abnormal Posture | Limbs, such as arms and legs, are bent and stretched in strange ways. |
| Pain and discomfort | Pain caused by muscle tightness and spasms. |
| Contracture | Muscles become stiff and joints become permanently contracted, as if they were made of stone, over a period of time. |
| Interference with daily tasks | Difficulty doing even simple tasks like getting dressed, bathing, or eating on your own. |
What are the causes of spasticity?
As I said before, the main reason for this is some damage to our central nervous system, that is, the brain or spinal cord. There are several possible causes for this damage.
- Spinal cord injury: Damage to the spine caused by an accident.
- Multiple Sclerosis (MS): A disease that damages the nerve sheaths.
- Cerebral Palsy: A condition caused by damage to the brain during childhood.
- Stroke: Damage to brain cells due to interruption of blood flow to the brain.
- Brain or head injury: An impact to the head from an accident.
- Amyotrophic Lateral Sclerosis (ALS): A disease that gradually weakens the nerve cells that control movement.
- Spasticity can also be caused by several other genetic conditions.
How is spasticity treated?
The best thing is that there are many ways to treat this condition. While spasticity cannot be completely cured, there are many things you can do to manage symptoms, reduce pain, and make your life easier.
The most effective way to treat this is to work with a multidisciplinary team of doctors, not just one person. This team may include a neurologist, a physiatrist, a physical therapist, and an occupational therapist.
Non-surgical treatments
These are the first treatments to be used.
- Physical therapy: This mainly involves stretching and strengthening exercises for tight muscles, as well as training in walking and standing.
- Occupational therapy: This involves exercising the muscles in your hands so that you can do your daily tasks, such as dressing, bathing, and cooking, on your own.
- Speech therapy: If spasticity affects the muscles in your face, mouth, and throat, this therapy can help you overcome difficulty speaking and swallowing.
- Medications: There are medications that can be used to relax muscles. For example, medications like `(Baclofen)`, `(Tizanidine)`, `(Diazepam)`. However, these can sometimes cause side effects such as drowsiness and weakness. Therefore , it is essential to use these medications only as prescribed by a doctor.
Botox injection treatment
Yes, it's the same `(Botox®)` you've heard of. This is not only a cosmetic treatment, but also a very effective treatment for spasticity. What you do here is inject this injection directly into the tight muscles. Then those muscles relax. The effect of this lasts for about 3 months.
Surgery (surgery)
These are usually used when the illness is more severe.
- ITB Therapy: This involves surgically implanting a small pump under the skin of the abdomen, which releases a specific dose of a drug called ``(Baclofen)`` directly into the fluid surrounding the spinal nerves. It has fewer side effects than taking it by mouth.
- Selective Dorsal Rhizotomy (SDR): This is a slightly more complex surgery. In this, only the nerve roots that are causing the problem are selected and cut. This can reduce severe leg stiffness.
- Orthopedic surgery: Surgery to restore joints and tendons that have become stiff and stiff due to spasticity.
When should you definitely see a doctor?
This is very important.
If you have never had any problems before and suddenly experience muscle stiffness or twitching, it could be a sign of brain or spinal cord damage, so see a doctor without delay. If necessary, go to the Emergency Department (ETU).
If you already know you have spasticity, talk to your doctor if the following things happen:
- If the stiffness and cramping are increasing day by day.
- If the pain increases.
- If you are having increased difficulty performing daily tasks.
Take-Home Message
- Spasticity is a condition in which muscles tighten and twitch uncontrollably, caused by damage to the brain or spinal cord.
- The severity of this can range from mild discomfort to severe pain that makes daily activities impossible.
- Don't worry. There are many successful treatments, from physical therapy to surgery.
- By talking to your medical team, you can develop the best treatment plan for you.
- If you experience any new symptoms or your condition worsens, seek medical advice without delay.
