Today we are going to talk about such a serious, but often silent, disease. In medical terms, we call this 'Ascending Aortic Aneurysm'.
Simply put, what is this Ascending Aortic Aneurysm?
This may sound like a complicated name, but it's very simple to understand.
Let's break this name down first.
- Aorta: This is what I mentioned earlier, the largest and most important blood vessel in our body. It starts from our heart.
- Ascending Aorta: This is the ascending part of the aorta closest to the heart.
- Aneurysm: This is the real story. This is when a weak spot in the wall of a blood vessel bulges out. It's like a "bubble" coming out of a tire on a car.
Simply put, an Ascending Aortic Aneurysm is a condition in which the wall of the first part of the aorta that starts in our heart becomes weak and bulges like a balloon. This condition is sometimes called an Ascending Thoracic Aortic Aneurysm (ATAA) because it occurs in the chest area.
The biggest danger is that this swollen area can tear or rupture at any time. If that happens, it can cause severe bleeding inside the body, which can be very serious and even life-threatening.
Who is most at risk of developing this condition?
Although this condition can occur in anyone, some people are at higher risk. Let's take a look at who they are. Think about whether these facts apply to you.

About 60% of all thoracic aortic aneurysms occur in the ascending aorta. According to statistics, this condition affects about 10 out of 100,000 people each year.
Why is this happening? What are the reasons?
Anything that weakens or damages the walls of a blood vessel can cause this. As we age and with heart disease, our arteries lose their elasticity. Then, they can't withstand the pressure of blood flowing through them, and they start to bulge at the weak points.
There are several main reasons that affect this situation:
- Atherosclerosis: We simply call this "fatty deposits in the blood vessels." In this way, things like cholesterol can build up in the walls of the blood vessels, hardening them and weakening them.
- Aortitis: This is inflammation of the aorta. It can be caused by certain infections or immune system problems.
- Bicuspid aortic valve disease: As mentioned earlier, this is a congenital defect of the heart valve. People with this condition are at a much higher risk of developing aortic dissection.
- Genetic Disorders:
- Marfan Syndrome: A disease that weakens the body's connective tissues.
- Loeys–Dietz Syndrome: Another genetic disease that can cause enlargement of the aorta.
- Turner Syndrome: A genetic condition that only affects females and can cause problems with the cardiovascular system.
- Traumatic Injuries: A severe accident or blow to the chest can damage and tear the wall of the aorta.
What are the symptoms of this? How do you recognize it?
This is the silent and dangerous side of this disease. Many people don't have any symptoms. You can go unnoticed until the aneurysm gradually grows.
However, as the aneurysm grows larger or presses on nearby organs, certain symptoms may occur. If you experience any of these symptoms, it is important to talk to your doctor right away.
- Chest pain or pain in the upper back area: This can be a deep, squeezing pain.
- Coughing or Wheezing: These symptoms may occur if the aneurysm is compressing the airway.
- Difficulty swallowing: This may be due to the aneurysm pressing on the esophagus.
- Hoarseness: The voice may change due to compression of a nerve that controls the vocal cords.
- Shortness of breath.
How does a doctor accurately diagnose this disease?
Often, this condition is discovered incidentally during testing for another condition. For example, a doctor may find an enlarged aorta while taking a chest X-ray for another reason.
If the doctor suspects this, he or she will order various tests to confirm it and determine the exact size and location of the aneurysm.

How is it treated?
Treatment depends on factors such as the size of the aneurysm, how fast it is growing, and the cause.
For small aneurysms (less than 5.5 cm)
If the aneurysm is smaller than 5.5 centimeters and you don't have other special risk factors (such as Marfan Syndrome), you usually won't be referred for surgery right away. Instead, your doctor may recommend the following:
- Medications: Medications are given, especially to control blood pressure. When blood pressure is lowered, the pressure on the blood vessel walls is also reduced.
- Constant monitoring:A scan is done about every 6 months to see if the aneurysm is getting bigger. If it's not growing rapidly, you can check it every few years.
- Lifestyle changes:
- If you smoke, it is imperative to stop immediately .
- Eating a healthy diet.
- Engage in light exercise that does not put too much pressure on the blood vessels. You should talk to your doctor about this and seek advice.
For large aneurysms (greater than 5.5 cm)
If the aneurysm is larger than 5.5 centimeters, surgery is usually recommended, as the risk of rupture is much higher after that size.
However, for people with conditions like Marfan Syndrome, a Bicuspid Aortic Valve, or a family history of aneurysms, doctors may recommend surgery even if the aneurysm is smaller. This is because they are at higher risk of rupture, even if the aneurysm is small.
The surgery involves opening the chest and cutting out the weak part of the aorta that is bulging. Then , a graft is inserted in its place. This is made of a strong synthetic material. During this surgery, the work of the heart and lungs is temporarily transferred to a machine called a ``cardiopulmonary bypass machine.`` Sometimes, if there is a heart valve defect, that surgery is also done at the same time.
How is the condition after treatment? (Prognosis)
This is a problem for many people.
Studies have shown that 79% of people who undergo elective surgery, or 79 out of 100, live in good health for at least 10 years after treatment.
However, if an aneurysm ruptures or bursts, the situation is very serious. If emergency surgery is not performed within 48 hours, about half of such patients will not survive. This shows how important it is to recognize this condition early and seek appropriate treatment, even if there are no symptoms.
What can we do to protect ourselves from this situation?
Although we cannot control some genetic causes, there are many things we can do to reduce the risk.
- A healthy lifestyle: Eating a balanced diet and exercising regularly.
- Control risk factors: Keep your weight, blood pressure, and cholesterol levels under control. Take the medications your doctor prescribes for these exactly as prescribed.
- Get rid of bad habits: If you smoke, stop immediately. Avoid excessive alcohol consumption.
