Are you always tired? Could this be Chronic Fatigue Syndrome?
Table of Contents ▾

There are days when, no matter how much sleep we get, no matter how much rest we get, we feel like we have no energy left in us. It’s as if someone has sucked all the energy out of us. If this condition has been a part of your life for months, don’t dismiss it as just normal fatigue. Today, we’re going to talk about such long-term fatigue that doesn’t go away with rest. In medicine, we call this Myalgic Encephalomyelitis/Chronic Fatigue Syndrome, or (ME/CFS) for short.

Why is this disease so difficult to diagnose?

One of the biggest challenges with this condition is that there is no specific blood test, X-ray, or scan to confirm that it is this. Also, the main symptoms of this condition, such as extreme fatigue, difficulty doing even the smallest things, and problems with memory and attention, are also symptoms of many other diseases. Therefore, it is difficult for even a doctor to immediately say that this is ME/CFS. It takes a while, because you first need to make sure that there are no other diseases.

Simply put, ME/CFS is a diagnosis that remains after everything else has been ruled out. So this journey requires some patience.

First thing you should do: see a doctor

If you have any suspicions that you may have these symptoms, the best thing to do is to see your family doctor (GP) immediately . Research has shown that the earlier this condition is diagnosed and treated, the better the outcome.

The doctor will ask you a lot of details about your health condition.

  • When did you start feeling this tired?
  • When does it increase?
  • What types of medications do you take? (You should also mention the regular medications you buy from the pharmacy.)
  • Are there any additional vitamins or herbal supplements that you take?

All of this is important. Because sometimes fatigue can be a side effect of other medications you're taking. Also, some herbal products that you think are "natural" can affect your body. Your doctor will likely examine you thoroughly and, if necessary, run blood, urine, and other tests. All of this is done to first determine if you have another treatable condition.

What other diseases have similar symptoms?

There are a number of other conditions that can have similar symptoms to ME/CFS. A doctor will first try to rule out these conditions.

  • "Mono" (Mononucleosis): This is also a viral infection that causes extreme fatigue.
  • Lyme disease:A tick-borne disease that causes joint pain and fatigue.
  • Lupus: A disease caused by a problem with the body's immune system.
  • Multiple Sclerosis: A disease that affects the nervous system.
  • Fibromyalgia: A condition that causes pain and fatigue throughout the body.
  • Sleep disorders: Conditions that cause insomnia.
  • Depression: Depression can also cause severe physical fatigue.

Imagine, if you have a treatable disease on this list, your fatigue could be completely cured with the right medication. That's why ruling out other diseases like this is so important.

How is ME/CFS diagnosed?

After ruling out all other conditions, your doctor will take a closer look at your symptoms. To be diagnosed with ME/CFS, you need to meet a set of specific symptoms. To help you understand these more clearly, see the chart below.

Symptom category Description
The main requirement Having severe fatigue that lasts for more than 6 months , does not improve with bed rest, and significantly reduces your daily activities (work, education, social activities).
In addition, the following three basic characteristics must be present:
1. Post-Exertional Malaise Symptoms worsen after minor physical or mental exertion (e.g., going to the store, reading a book). This condition can last for hours or days.
2. Unrefreshing SleepNo matter how long you sleep, you wake up in the morning feeling rested and refreshed. It feels like you've been awake all night.
3. Decreased ability to perform daily tasks Inability to perform normal daily activities (going to work, doing household chores, studying) for more than 6 months due to fatigue.
In addition to all of the above, at least one of these two characteristics must be present:
1. Thinking and memory problems (Cognitive Impairment) Difficulty concentrating, difficulty remembering things, and a feeling of "fog" in your thoughts (Brain Fog).
2. Orthostatic Intolerance Dizziness , faintness, and weakness may occur when you suddenly stand up from a sitting or lying position. Blurred vision or spotting may also occur.

It can take some time to get a diagnosis. During that time, ask your doctor how to manage your symptoms. He or she will start you on treatments and continue to monitor you to see if they are working.

💡 Take-Home Message

  • Not normal fatigue: If you have severe fatigue that lasts for more than 6 months and is not relieved by rest, don't ignore it.
  • See a doctor immediately: It is essential to seek medical advice for an accurate diagnosis and to ensure that there are no other dangerous medical conditions.
  • There is no specific test: There is no specific test to diagnose ME/CFS, and it is a diagnosis made after ruling out other diseases.
  • Be patient: The diagnostic process can take time. Work closely with your doctor to manage your symptoms during this time.
  • You are not alone: This is a situation that many people around the world experience. With the right management and support, life can be better.
