Have you undergone surgery for thyroid cancer? Or are you undergoing treatment for it? If so, your doctor may have told you about a special injection as the next step. Often, when you hear the name of this injection, you may feel a little curious, and maybe even a little scared. But there is no reason to be afraid. Today we are talking about this medicine called Thyrotropin Alfa, which is very important in the treatment of thyroid cancer.
Simply put, what is Thyrotropin Alfa?
Thyrotropin Alfa is a man-made protein produced in a laboratory. What makes it special is that it works in the same way as a natural hormone produced by our body's brain. That hormone is Thyroid Stimulating Hormone (TSH) .
Think of the thyroid cells in our body as little sleeping workers. The TSH hormone is like a manager who "wakes" those workers up and gets them to work. So, this drug called Thyrotropin Alfa artificially performs the same function of the TSH hormone. This drug is most commonly available on the market under the name Thyrogen .
Why do we need this vaccine?
There are two main reasons for using this vaccine. Both are very important steps in the thyroid cancer treatment process.
1. Check to see if the cancer has come back (for diagnosis)
After surgery to remove the thyroid gland due to thyroid cancer, sometimes a few cancer cells that are too small to be seen with the naked eye may remain in the body. It is very important to find these. This vaccine helps in two ways:
- Radioactive Iodine Scan: This scan is performed before the Thyrotropin Alfa injection is given. This causes thyroid cancer cells that are hidden somewhere in the body to become "awakened" and active. The dose of radioactive iodine given is then absorbed by these active cells. When viewed on the scan, the locations of the cells that have absorbed the iodine can be clearly identified.
- Thyroglobulin (Tg) blood test: Thyroglobulin is a protein produced only by thyroid cells. After the Thyrotropin Alfa injection, if there are any remaining cancer cells, they will start to produce this Tg protein. A subsequent blood test can measure this Tg level and help the doctor determine whether or not there are any remaining cancer cells.
2. Destroy remaining cancer cells (for treatment)
Sometimes, a treatment called radioactive iodine ablation is given to destroy any remaining cancer cells after surgery. For this treatment to be successful, the cancer cells need to absorb the radioactive iodine well. The Thyrotropin Alfa injection "awakens" the cancer cells and allows them to absorb the dose of iodine given for the treatment.
Simply put, this vaccine works like a spy that shows the way to find enemies and attack them.
Things you need to tell your doctor before taking this medicine
It is very important for your safety to discuss these issues honestly and clearly with your doctor before starting this treatment.
- If you have an allergic reaction to Thyrotropin Alfa, other thyroid products, hormones, other medications, foods, or preservatives.
- If you have heart disease .
- If you have kidney disease .
- If you know that the cancer has spread (metastasized) to other parts of the body.
- If you smoke .
- If you are pregnant, trying to get pregnant, or breastfeeding .
Based on this information, the doctor will determine the most appropriate and safest treatment plan for you.
How is the treatment given?
This is not a home remedy. This is an injection into a muscle . It is usually given in the buttock or upper arm. It is given to you by a trained doctor or nurse in a hospital or clinic.
Important: It is very important that you get your vaccinations on time. If for some reason you are unable to go to the hospital on the scheduled date, call your doctor immediately and let them know.
If you think you have accidentally taken an overdose, do not panic and immediately call the National Poisons Information Centre at the Colombo National Hospital , or go to the Emergency Treatment Unit (ETU) of the nearest hospital.
What are the possible side effects of the medication?
Like all medicines, Thyrotropin Alfa can cause some side effects, although not everybody gets them. The table below provides a clearer picture of these.

|---|---|
| Serious side effects that should be reported to your doctor immediately | |

| Chest Pain | A feeling or pain as if the heart is being squeezed. |
| Symptoms of a Stroke | Sudden severe headache, confusion, difficulty speaking or understanding, numbness or weakness in the face, arm or leg, difficulty walking, changes in vision. |
| Things that are usually not serious, but should be reported to the doctor if they persist | |
| Headache | A general headache may occur. |
| Nausea and vomiting | Stomach upset, nausea, or vomiting may occur. |

Does it interact with other medications?
This medicine does not usually interact with other medications. However, the most important thing here is to inform your doctor about all the medications you are taking, including vitamins, herbal remedies, and supplements. That way, your doctor can prevent any potential problems.
