What is the Pulmonary Valve? Let's understand it simply!
Think of your heart as a small house with four rooms. In order for blood to flow from one room to another, there must be doors, right? That's what the pulmonary valve is like.
It's one of the four valves in the heart. To be precise, it's
the 'door' that takes oxygen-poor blood from the lower right chamber of your heart (called the ``right ventricle'') to the ``main pulmonary artery'', the main blood vessel that carries oxygen-poor blood to the lungs. When this blood reaches the lungs, it picks up oxygen. Then, that oxygen-rich blood travels to the rest of the body. So, can you imagine how important it is for this door to work properly? The pulmonary valve, along with another valve in our heart, the ``aortic valve'', are collectively called the
``semilunar valves'' . This is because the flaps (like the doorknobs) on these valves are shaped like crescents. These crescent valves control the flow of blood from the lower chambers (ventricles) of our heart to the larger
blood vessels (arteries). You may also hear this called the Pulmonic valve. They are both names for the same thing.
So what exactly does this pulmonary valve do?
The main function of this pulmonary valve, as we mentioned earlier, is to control the flow of oxygen-poor blood from the right ventricle of the heart to the main pulmonary artery, which goes to the lungs. Let's take a look at how this happens in a little more detail:
- Every time your heart beats, this pulmonary valve opens and closes, just like a door.
- When your right ventricle contracts (this is called the ``systolic phase'' in medical terms), the pulmonary valve opens. This is when blood begins to flow out of the heart, towards the lungs.
- Then, when the heart 'relaxes' for a while (this is called the ``diastolic phase'' or the relaxation phase), the pulmonary valve closes. This is very important. Because if it doesn't close like this, blood that has gone to the lungs can leak back into the heart. That's not good.
- So, it's when this valve is closed that the right ventricle fills with the blood it needs to contract the next time.
Think of it like a water tap. You open it when you need it and take in water, and you close it when you're done. Otherwise, the water goes to waste. The pulmonary valve also controls blood flow in the same way.
The special role of the pulmonary valve in the Ross procedure
In addition to normally controlling blood flow, this pulmonary valve can also perform another special function. That is, it can be used as a replacement when the other important valve in our heart, the `(aortic valve),` becomes diseased and cannot work properly. This is called the
`(Ross procedure)` . This is a slightly more complex surgery. It is done in some children and adults under the age of 60 with `(
Aortic Valve Disease )`. In this surgery, a surgeon removes your diseased `(aortic valve)` and replaces it with your own healthy pulmonary valve. Then, a pulmonary valve from someone else (
donor ) is transplanted into you. Your own pulmonary valve is a good replacement for your aortic valve because the structure and function of both are very similar. That's why it lasts a long time.
Where exactly is this pulmonary valve located?
This is inside your heart. As we mentioned earlier,
this is like a connection between your right ventricle and the pulmonary artery, the main artery that carries blood to your lungs. It's located to the right of, and slightly in front of, the aortic valve, which is another major valve in the heart.
What is the structure, that is, the shape, of the pulmonary valve?
The pulmonary valve
has three main cusps or flaps. The opening and closing of these flaps controls the flow of blood. These three flaps are called the anterior cusp, the left cusp, and the right cusp. These flaps are attached to a slightly stronger, fibrous ring of tissue called the annulus. When the heart relaxes (that is, during the diastolic phase), these three flaps fit together tightly and completely close the valve.
This prevents blood from flowing backward into the right ventricle (regurgitation). What are the diseases and conditions that affect the pulmonary valve?
Pulmonary valve disease can occur in many different ways.
Most often, these are congenital heart conditions. This means that you or your baby may have these problems at birth. However, rarely, problems with this valve can develop later in life. This is usually due to other conditions, such as
pulmonary hypertension . Some of the conditions that can affect the pulmonary valve include:
- Pulmonary atresia: This is a heart condition that is present at birth. This happens when the baby's pulmonary valve doesn't develop properly, or its opening is blocked by tissue. This causes blood to flow to other parts of the baby's heart. This causesAt birth, the baby may turn blue ( cyanosis ) (meaning low oxygen levels).
- Tetralogy of Fallot: This is also a congenital heart disease. This condition affects about 1 in 2,500 babies born in the United States. In this case, the baby has four abnormalities in the heart. One of these abnormalities is pulmonary stenosis (narrowing of the pulmonary valve and the main pulmonary artery). This is a serious condition because it limits the amount of oxygen-rich blood that circulates throughout the baby's body. Babies with this condition will definitely need surgery.
- Pulmonary valve stenosis: This is a narrowing of the valve. This is also usually present at birth. It is also often seen in babies with Tetralogy of Fallot. There are minimally invasive procedures that can be used to open the valve as a treatment. Some people may also need to have the valve replaced. It is very rare for someone born with a healthy pulmonary valve to develop pulmonary stenosis later in life.
- Absent pulmonary valve: This is also a congenital heart condition. It affects about 1 in 50,000 babies. It is most common in babies with Tetralogy of Fallot. This is when the leaflets/flaps of the baby's pulmonary valve are missing or do not develop properly. This causes the valve opening to narrow (stenosis) and blood to leak backward (regurgitation). This causes blood to leak back into the right ventricle from the baby's main artery to the lungs (pulmonary trunk). This condition also needs to be treated early in life.
- Pulmonary (pulmonic) valve regurgitation: This is when blood leaks through the valve. This is usually caused by pulmonary hypertension in older people or by congenital heart disease. It can also occur in some people who have had surgery for Tetralogy of Fallot. This happens when the pulmonary valve does not close completely each time the heart beats. As a result, when the heart relaxes, some blood leaks back into the right ventricle from the pulmonary artery. Most people have mild symptoms. However, some people may have moderate or severe symptoms that require treatment. If this type of leak persists for a long time, it can cause dilation and failure of the right ventricle, as well as swelling (edema).
Is it possible to live without a pulmonary valve?
Oh no.
The pulmonary valve is a very important part that sends blood to the lungs and helps that blood get oxygen.We cannot live without a properly functioning valve. Babies and children with serious pulmonary valve conditions can have a shortened life expectancy if they are not treated properly. How do I keep my pulmonary valve healthy?
Heart valve problems can't always be prevented, especially those that are present at birth. But,
in general, following a heart-healthy lifestyle can go a long way in keeping your valves and heart in good working order. Here are some things that can help:
- Quit smoking, vaping, and all tobacco products completely. Ask your doctor for help quitting.
- Limit alcohol consumption , or stop it completely.
- Eat a heart-healthy diet. (We'll talk about this in a separate article!)
- Exercise regularly as recommended by your doctor. Most people should do about 150 minutes of moderate-intensity exercise per week, such as walking.
- Control conditions like high blood pressure and high cholesterol.
- Take the medications prescribed by the doctor exactly and on time.
- Have a medical checkup at least once a year.
- If you are pregnant, be sure to go to the appropriate obstetrics and gynecology clinics and get the necessary care. Doing so can reduce the risk of congenital heart disease in your newborn baby.
