One Monday, during a busy time in my clinic, I first met Ryan. Ryan is a 26-year-old software developer. He came into the room very shy and timid. When he came in and sat down in his chair, I could tell he was very stressed out about this health issue.
"Dr. Priya, this is a little hard for me to say," Ryan began, his voice trembling as he interlaced his fingers. "For the past few days, I've been having a burning sensation when I urinate. I've also been having a white or yellow, pus-like discharge from my urethra."
I smiled and calmed him down. "Ryan, you don't have to be ashamed or afraid of anything. Sexually transmitted diseases are just like the common cold. The best thing to do when you have a problem like this is to see a doctor and talk openly. Let's see what's bothering you."
What is gonorrhea?
After carefully examining Ryan's symptoms and his medical history, I realized that it was very likely gonorrhea . Gonorrhea is a very common bacterial infection that is transmitted through sexual contact. It is caused by the bacterium Neisseria gonorrhoeae .
In men, this bacteria usually infects the urethra . However, depending on the nature of sexual activity, such as oral sex or anal sex, this infection can also occur in the throat, eyes, or rectum.
According to the World Health Organization (WHO), more than 82 million people worldwide are infected with gonorrhea each year, making it one of the most common sexually transmitted infections (STIs) caused by bacteria.
🚨 Red Flags that require immediate medical attention
Since gonorrhea infection can be life-threatening if it spreads throughout the body, you should seek immediate hospitalization and treatment if you experience the following symptoms:
- Severe pain and swelling in the joints along with a high fever: This is a symptom of Disseminated Gonococcal Infection (DGI) , a condition caused by the spread of bacteria into the bloodstream.
- Small pus-filled blisters or red rashes appear in places on the skin.
- Severe pain and swelling in the testicles (Epididymitis).
- Unbearable severe pain in the lower abdomen or pelvic area in women (PID symptoms).
Gonorrhea symptoms
Many people (especially women) who contract gonorrhea do not experience any external symptoms.
1. Characteristics that occur in males:
- Severe burning during urination: Ryan described the unbearable pain as if a razor blade was cutting through the urethra from the inside.
- Urinary discharge: White, yellow, or green pus-like discharge from the urethra.
- Testicular pain: You may feel mild pain or swelling in the testicles or groin area.
2. Symptoms that occur in women:
More than 50% of women do not experience any symptoms, and if symptoms do occur, they may be similar to those of a common urinary tract infection:
- Pain and burning during urination.
- Vaginal discharge is more abundant than usual or its color changes.
- Vaginal bleeding outside of menstruation or severe pain during sexual intercourse.
- Persistent pain in the lower abdomen (caused by Pelvic Inflammatory Disease - PID ).
Diagnosis and treatment methods
1. Tests that diagnose the disease
The following tests are performed to accurately confirm whether you have gonorrhea:
- Urine Test: A very simple, painless test.
- Swab Test: A swab is taken from the urethra, vagina, throat, or anus. Modern genetic tests such as Nucleic Acid Amplification Tests (NAATs) can detect the bacteria with more than 95% accuracy.
2. Medications given for treatment
Since gonorrhea is a bacterial infection, it is 100% curable with antibiotics:
- Ceftriaxone vaccine: Usually given as a single dose injected into the buttock muscle.
- Doxycycline or Azithromycin: Your doctor may prescribe these pills if another sexually transmitted infection (e.g. chlamydia) is suspected.
⚠️ Important: Currently, antibiotic-resistant strains of gonorrhea (Super Gonorrhea) are spreading worldwide. Therefore, it is imperative to take the full dose of medication prescribed by the doctor for the full number of days.
Do's & Don'ts
| ✔️ Things to do (Do) | ❌ Don'ts |
|---|---|
| If you are infected, advise your sexual partner to get tested and treated immediately. | Do not engage in any form of sexual intercourse until you have received treatment and are completely healed (at least 7 days). |
| If you are sexually active, get blood and urine tests for sexually transmitted diseases (STIs) at least once a year. | Do not stop taking the antibiotics prescribed by your doctor just because your symptoms have subsided. |
| Use condoms correctly every time you have sex (this reduces the risk of infection by 90%). | Do not be embarrassed to take antibiotics at the pharmacy without seeking medical advice. |
❓ Some other questions you may have (FAQ)
How long does it take for symptoms to appear after contracting gonorrhea? ▼
Symptoms, such as urinary tract infection, usually appear between 2 and 10 days after the bacteria enter the body. However, some people may not experience any symptoms for several weeks.
Can gonorrhea infection cause infertility? ▼
Yes, if left untreated, blockage of the vas deferens in men and damage to the fallopian tubes in women (due to PID) can lead to permanent infertility.
Is it possible to get gonorrhea again once you've been cured? ▼
Yes, even if gonorrhea is cured, the body does not build up natural immunity. If you have unprotected sex with an infected partner again, you can get infected again any number of times.
Can gonorrhea be cured naturally at home? ▼
No, to kill the bacteria, you must take the proper antibiotics prescribed by a doctor. Self-treatment methods cannot cure the disease.
