Let's talk about Goodpasture Syndrome, which damages both the lungs and kidneys at the same time.

Imagine what would happen if our own army, our immune system , which is supposed to protect our body from disease , suddenly misunderstands and starts attacking our own good organs? That is one such rare but very serious condition called Goodpasture Syndrome. In this disease, our immune system mainly damages two of the most important organs in our body, the lungs and kidneys . It is very important to be aware of this because it can be life-threatening if not recognized and treated quickly.

What exactly is Goodpasture Syndrome?

Simply put, Goodpasture Syndrome is an autoimmune disease. That is, our body's defense system, the immune system, goes haywire and attacks our own cells and tissues.

In this disease, the immune system makes antibodies against a special protein called collagen , which is found in our lungs and kidneys. Think of this collagen as the glue that holds parts of our body together. So when these antibodies attack that collagen, the lungs and kidneys start to get severely damaged.

Although this is a very rare disease, it is essential to recognize and begin treatment as soon as symptoms appear, otherwise the consequences can be very serious.

What causes this situation?

Doctors still don't fully understand why our immune system mistakenly attacks its own organs in this way. However, there are several factors that are thought to play a role.

  • Genetic influence : Since this disease runs in some families, it is believed that there may be a genetic predisposition to it.
  • Environmental factors: Exposure to certain chemicals , such as hydrocarbon solvents and herbicides like paraquat.
  • Metal dust: Constant exposure to metal dust.
  • Drugs: Use of drugs such as cocaine.
  • Smoking: Tobacco smoking is a major risk factor for this.
  • Viral infections: This condition can occur after some viral infections.

This disease is most commonly seen in young men, especially those between the ages of 20 and 30 and those over the age of 60.

What are the symptoms? How do you recognize this?

The early stages of this disease show very general, vague symptoms. So sometimes it may not seem like it's something serious. But this condition progresses very quickly.

See the table below for what these symptoms are.

Characteristic type Symptoms that can be seen
Initial and common features
  • Extreme fatigue and exhaustion
  • Nausea and vomiting
  • Pale skin (due to anemia)
  • Difficulty breathing
When the lungs are affected
  • Gradually increasing difficulty breathing
  • Persistent cough
  • Coughing up blood (this is a very dangerous sign)
  • When the kidneys are affected
  • Decreased urine output
  • Blood in the urine (red or brown urine)
  • Foamy urine
  • Swelling of the legs
  • High blood pressure
  • Burning or discomfort when urinating
  • Back pain below the ribs
  • Swelling of hands and feet
  • Although this disease can cause life-threatening bleeding from the lungs, long-term lung damage is usually minimal. However, the most serious consequence of this disease is kidney failure . If this happens, you may need lifelong dialysis or a kidney transplant.

How does a doctor diagnose this disease?

If you experience one or more of the symptoms we've discussed, don't delay . See a doctor as soon as possible. Especially if you're coughing up blood, having difficulty breathing, or passing blood in your urine, it's an emergency.

The doctor may order several tests to confirm the diagnosis:

    • Urinalysis: This test checks for high levels of protein and red blood cells in the urine. This can help determine if the kidneys are damaged.
    • Blood test: Your blood is checked for specific antibodies that attack the lungs and kidneys.
    • Chest X-ray: This can look for damage to the lungs, such as unusual white spots caused by bleeding.
    • Biopsy: Sometimes a small piece of tissue is taken from a kidney or lung and examined under a microscope. This can confirm whether Goodpasture Syndrome antibodies are present and how much damage has occurred to the kidneys.

What are the treatments for this?

Once this disease is diagnosed, treatment should begin very quickly and intensively. The main goals of treatment are:

    • Fighting harmful antibodies.
    • Controlling excess fluid accumulation in the body.
    • Controlling high blood pressure.
    • Stopping damage to the lungs and kidneys.

    Treatment usually involves the following:

    1. Immunosuppressive drugs: Drugs like cyclophosphamide and corticosteroids are given. These drugs work by reducing the activity of our immune system and stopping the production of harmful antibodies. Sometimes high doses of steroids may have to be given intravenously to control bleeding in the lungs.

    2. Plasmapheresis: This is a slightly more complicated procedure. Simply put, it involves taking a small amount of your blood, putting it in a special machine, and separating out the liquid part, called plasma, that contains the harmful antibodies. Then, the remaining good parts of the blood (red and white blood cells) are mixed with another plasma substitute and returned to your body. This removes the dangerous antibodies from the blood. This treatment may need to be done daily for several weeks.

This treatment may take 6 to 12 months. It can take anywhere from a few weeks to two years to fully recover. During this time, some patients may need supplemental oxygen or even a ventilator. Blood transfusions may also be necessary.

    Take-Home Message

    • Goodpasture Syndrome is a rare but very serious condition in which our own immune system attacks the lungs and kidneys.
    • Symptoms such as coughing up blood, difficulty breathing, blood in the urine, and swelling of the body are very important warning signs.
    • If you have any of these symptoms, do not delay and see a doctor immediately . The sooner treatment is started, the better the results.
    • Treatment includes medications that control the immune system and methods such as plasmapheresis, which purifies the blood.
    • If you or someone you know has any doubts or questions about this, talk openly with your doctor about it.
