Learn about Amniotic Fluid Embolism, a dangerous condition that can occur during pregnancy

Are you about to give birth? Or is someone in your family getting ready to become a mother? Pregnancy is a wonderful time filled with so much hope, but today we will talk about a rare, but very dangerous, life-threatening condition that can occur during this time. This is called amniotic fluid embolism . Don't be alarmed by the name, this is a very rare condition. But it is very important to be aware of it.

What exactly is amniotic fluid embolism (AFE)?

Simply put, this is when the amniotic fluid that surrounds the baby while it is in the womb, or the baby's cells, hair, etc., get into the mother's bloodstream. Think about it, there is usually a natural barrier between the mother's and baby's blood systems. In this situation, somehow that barrier is broken, and the baby's blood mixes with the mother's blood.

This causes a severe allergic reaction in the mother's body. Suddenly, without warning , many of the body's systems begin to malfunction. This condition can occur shortly before, during, or after the baby is born.

This condition is very rare, affecting between 1 and 12 per 100,000 live births. But if it occurs, it is a very serious threat to the mother's life and requires immediate treatment .

What are the causes and risk factors for this condition?

In fact, doctors still don't know exactly what causes it. Because it comes on so suddenly, it's hard to say for sure what causes it. However, some studies have identified several risk factors that increase the likelihood of developing the condition.

Let's see what it is.

Risk Factor Simple Explanation
Age The mother's age at the time of delivery is 35 years or older.
Operative Delivery In cases where a baby is delivered by Caesarean section, forceps, or vacuum, the barrier between mother and baby may be affected.
Inducing labor (Induced Labor) Although there is a belief that some methods used to artificially induce labor may increase the risk, research on this is still ongoing.
Placental Abnormalities Conditions such as the placenta being positioned low in the uterus, covering the cervix (placenta previa) or the placenta detaching from the uterine wall during delivery (placental abruption).
Preeclampsia A condition that occurs during late pregnancy and is characterized by symptoms such as high blood pressure, protein in the urine, and swelling of the extremities.
Increased amniotic fluid (Polyhydramnios) The amount of amniotic fluid around the baby is much higher than normal.

What are the symptoms of this?

The most dangerous thing about this condition is that the symptoms appear suddenly, without even having time to think. The main symptoms are:

  • Sudden onset of severe difficulty breathing .
  • A sudden and dangerous drop in blood pressure (Hypotension).
  • Excessive and uncontrolled bleeding from the uterus or from a cesarean section wound.
  • Feeling very nervous, afraid , or anxious.
  • The baby's heartbeat is unusually slow or changes.
  • Feeling cold, shivering, and having seizures .
  • Heart failure or sudden loss of consciousness (Cardiac Arrest).

The cause of this excessive bleeding is the breakdown of the entire system that helps the body to clot blood. This is called Disseminated Intravascular Coagulation (DIC) in medicine. What happens in this is that the blood loses its ability to clot. As a result, the bleeding becomes so severe that it cannot be stopped. Also, difficulty breathing, damage to the lungs and the inability to breathe can occur, which is called Acute Respiratory Failure .

How is this condition treated?

Since this is a life-or-death situation, treatment begins in seconds. It is managed in a hospital's intensive care unit (ICU) or operating room. Here, it is not a single treatment, but a combination of several treatments depending on the patient's condition.

  • Immediate Delivery: Doctors decide to deliver the baby as soon as possible, considering the lives of both the mother and the baby.
  • Life-saving treatment (Resuscitation): If the mother becomes unconscious, oxygen is given while performing Cardiopulmonary Resuscitation (CPR) .
  • Mechanical Ventilation: If you have difficulty breathing, you are connected to a machine called a ventilator to help you breathe.
  • Blood pressure and heart monitoring: Special catheters are inserted into the body's major veins and arteries to closely monitor things like blood pressure and heart function, second by second.
  • Blood Transfusion: Due to excessive blood loss, large amounts of blood and blood components are given to replace lost blood and provide proteins (clotting factors) needed for blood clotting.
  • Specialized medications: Various medications are given to restore low blood pressure, improve heart function, and stop bleeding by causing the uterus to contract.

All of this is done with the primary goal of saving the mother's life. Although the survival rate for the mother in this situation is low, early treatment can increase the chances of survival. The baby's survival rate is much higher (about 70%).

Take-Home Message

  • Amniotic fluid embolism (AFE) is a rare but extremely dangerous condition that can occur during pregnancy.
  • This is when the amniotic fluid around the baby enters the mother's bloodstream. The exact cause of this has not yet been discovered.
  • Severe symptoms such as difficulty breathing, low blood pressure, excessive bleeding, and loss of consciousness suddenly appear.
  • This is a medical emergency. Immediate admission to the hospital's Intensive Care Unit (ICU) or Emergency Treatment Unit (ETU) is required for treatment, and life-saving treatment should be initiated immediately.
  • Don't be alarmed by this condition, as it is very rare. However, it is important to inform your doctor about any unusual symptoms you experience during pregnancy.
