We don't think much of a pimple on our face, especially on the sides of our nose or above our upper lip, a toothache, or a sinus infection, do we? But did you know that even very rare, common things like this can lead to a serious condition that can be life-threatening if not treated properly? Today we are talking about one such rare but extremely dangerous condition, " Cavernous Sinus Thrombosis ".
Simply put, what is Cavernous Sinus Thrombosis?
Imagine that under our brain, behind our eye sockets, there is a small hollow space. This is what we call the "Cavernous Sinus" in medicine. Through this passes a major vein that carries used or dirty blood from our face and head back to the heart.
So, Cavernous Sinus Thrombosis is when a blood clot forms inside this blood vessel and gets stuck. This blood clot blocks blood flow and increases pressure in that area. This pressure directly affects the brain, causing brain damage and in some cases, even death. This is so serious that about 30% of people who develop this condition die. Therefore, it is very important to be aware of this.
What are the symptoms of this dangerous condition?
Some of the symptoms of this disease appear suddenly. If you have one or more of these, you should be concerned.
The most important thing is, if a severe headache is accompanied by an eye problem (swelling, redness , change in vision), never ignore it.
Let's understand these symptoms more clearly from the table below.
| Symptom | What does this look like? |
|---|---|
| Severe headache | An unbearable headache. Sometimes it can come with tears in the eyes. |
| Changes around the eyes | Swelling, redness, and inflammation of one or both eyes. The eye may also appear to be protruding. |
| Eyelid drooping | A feeling of drooping eyelids in one or both eyes, making it difficult to open the eye fully. |
| Inability to move the eye | Inability to move the eye up, down, or to the sides. |
| High fever | High fever and malaise for no apparent reason. |
| Facial pain or numbness | Pain or numbness in the face or around the eyes. |
| Vision problems | Blurred vision, double vision, or complete loss of vision. |
| Confusion and seizures | Changes in memory, agitation, and coma can occur. Seizures are also rare. |
Why is this happening? What are the reasons?
The main reason for this is the spread of an infection. Usually, when an infection in our face, sinuses, or teeth cannot be controlled, the bacteria that caused the infection travel through the bloodstream and into the Cavernous Sinus we talked about earlier.
"Danger Triangle of the Face"
As doctors, we call the triangular area of the face from the tip of the nose to the corners of the mouth the "danger triangle." The blood vessels in this area are directly connected to the Cavernous Sinus inside the brain. Therefore , squeezing a pimple in this area and causing an injury can be very dangerous. If you do so, there is a high chance that germs on the surface of the skin will enter the blood vessels and go directly to the area surrounding the brain.
When an infection spreads, our body tries to stop it. As a result of that effort, a blood clot forms. But this blood clot is the biggest problem later.
There may be other reasons besides this:
- A serious head injury.
- Some people are at higher risk of developing blood clots. For example, women taking birth control pills, people with uncontrolled diabetes , and people with cancer are at slightly higher risk.
Tests to diagnose this condition
When you go to the hospital with symptoms like these, the doctor will examine you and, if they suspect this condition, they will quickly refer you for several tests.
- Brain scans: This is the most important thing. A CT scan or MRI scan may be done to look for things like blood clots and increased pressure in the brain.
- Blood tests : Blood tests are done to confirm whether there is an infection in the body.
- Lumbar Puncture: Sometimes, this test may also be done to see if the infection has affected the brain and spinal cord.
How is it treated?
Cavernous Sinus Thrombosis is a medical emergency. This means that as soon as symptoms appear, you should go to a hospital's Emergency Department (ETU) without wasting any time. Treatment is usually provided in the hospital's Intensive Care Unit (ICU).
Treatment mainly involves a few things:
- High-dose Antibiotics: To control the infection, strong antibiotics are given intravenously through a saline solution.
- Blood thinners: These medications are given to stop blood clots from getting bigger and to prevent further blood clots from forming.
- Corticosteroids: These types of medications are given to reduce swelling and pressure in the brain.
- Surgery: Sometimes, a minor operation may be needed to remove pus from where the infection started (e.g., in a sinus cavity).
The sooner these treatments are started, the greater the chance of saving a life. Therefore, it is important to be aware of these symptoms and take action quickly if something like this happens to you or someone you know.
Take-Home Message
- Cavernous Sinus Thrombosis is a very rare, but life-threatening, potentially fatal condition.
- The main reason for this is the uncontrolled spread of an infection in the face, teeth, or sinuses.
- Avoid squeezing pimples that appear in the "danger triangle" of the face (the area from the bridge of the nose to the corners of the mouth).
- An unbearable headache, along with swollen and red eyes, changes in vision, and a high fever are warning signs.
- If you have these symptoms, it is a medical emergency. Go to a hospital's Emergency Department (ETU) immediately and see your doctor.
